For a smooth and efficient appointment, please print out and complete all necessary forms prior to your scheduled appointment at our clinic. Please bring completed forms with you on the day of your appointment.
Completing these forms prior to your visit will decrease your wait and allow you ample time to complete all required insurance and health information at your convenience.
Instructions
- Download the “New Patient Form” by clicking the “Get New Patient Form” button below.
- Print out the “New Patient Form.”
- Completely fill out all requested information.
- Bring it with you to your first appointment with us.
Please Note: New patients who are being referred by another physician office or have any medical records including labs or imaging studies done at other medical facilities please have your records faxed to 770-357-2511. Address the Fax as New Patient Medical Records. Please request your records to be sent at least a week prior to your appointment with us.
